Notes:
a) “Note” was ‘Evaluation and Management‘ in previous LOINC versions for these codes
b) LOINC code availability and our principles favor not specifying SMD over setting, i.e. we code the document for the discipline of the author, not the location of the patient.
c) We will use Role = PA, NP, RN, etc… as needed.

d) Quebec is using option 3. 

The group mostly preferred Option 1.  However, one member wanted to think about it more.
